Welcome statement:

Welcome to the MDS Manager Study.  We look forward to your participation in the study entitled:  The impact of a mobile health application on technology engagement and communicative health literacy in older adult cancer survivors.

MDS Manager^TM is a health and life management mobile health application (mHealth App) tailored for patients and caregivers LIVING with Myelodysplastic Syndromes (MDS).

The features included in MDS Manager are intended to help individuals LIVING with MDS to actively participate in their own care by providing a variety of tools to track information related to their health, and quickly link the user to tailored resources.

You will be guided through a series steps over the 4 weeks study period.

1. The baseline questionnaire will take approximately 20-30 minutes and can be completed on your device or online via the MDS Manager Study Portal.
2. You will then be guided to explore the MDS Manager User Guide if you have not done this already.
3. You will receive periodic notifications/messages during the study to help guide you to resources tailored to your MDS profile. You can access these resources through your device, or can also visit the MDS manager Study portal to access your data and these resources on your home computer.  All connections are secured as described.
4. At the end of the 4-week study period you will be directed to complete an end-of study questionnaire. The end-of-study questionnaire will take approximately 20-30 minutes
