Insurance approval for Imetelstat/Rytelo

Has any one had success with insurance covering Imetelstat? Because it is still new we are getting pushback with the generic j code used for billing. We do not have Medicare, we have BCBS. Just wondering if anyone has appealed or successfully got insurance to cover it somehow? Thanks!
